Position Overview & Specifications
You will have a small cohort of patients to manage with support from the critical care therapies team. To work with and support those who have complex needs that may include physical, psychological, mental health and communication difficulties. You work closely with all MDT professionals ensuring joined up care of long-term rehabilitation patients post critical care admission. You will ensure that patients are cared for in a safe environment and that privacy and dignity is maintained at all times.
- Plan, deliver, evaluate, and progress complex therapy interventions, using advanced clinical judgement and decision-making skills in order to work towards and achieve identified Therapy/patient goals
- To formulate and deliver an individualised therapy care plan from referral on critical care to discharge/transfer of care, in order to facilitate a timely and safe discharge from hospital
- To communicate with patients, in such a way, that respects and takes into account the patient’s level of understanding.
- To work with and support those who have complex needs including physical, psychological, mental health and communication difficulties.
