We are looking for an innovative, passionate and professional individual with excellent interpersonal and organisational skills to join the Stroke Recovery Team in Cwm Taf Morgannwg, Wales - Bridgend. Supporting stroke survivors and families in the community with their stroke recovery, rebuilding their lives and rebuilding confidence.

Reporting to the Service Delivery Coach in Wales, you will have the ability to support Stroke Survivors to identify their goals. Using strong assessment skills and a person-centred approach, enabling them to maximise their recovery and improve communication.

Key Responsibilities will include:

  • Running and facilitating Communication Support sessions and other groups across the area.
  • Building relationships with external organisations and health and social care professionals.
  • Managing a team of volunteers who will support your work.

You will have:

  • A proven track record in health or social care.
  • Excellent IT skills to enable you to capture your work on a database and provide support through virtual means as required.
  • You will be organised and able to work on your independently, to manage your priorities to deliver a quality service.

This role requires extensive travel across a large geographical locality to visit people at home and support weekly in community groups. Candidates must be able to demonstrate how they can meet this requirement of the role.

Our services are contracted; we currently have funding for this contract until 31 March 2024.

To fulfil the role, you must be a resident of the UK and have the right to work in the UK.
