Position Overview & Specifications

Band 4 Community Support Workers – 1 Post full time 37.5hrs per week

Great opportunities exist with a vacancies arising for Community Support Workers in the Wandsworth Recovery Support Team. Successful applicants will have scope to develop as professionals in the field of Mental Health, in a relevantly small multi-disciplinary team. This Team works with adults predominately aged 18-75 who have severe and enduring mental health conditions in the Borough of Richmond.

The successful applicants will assist the qualified clinicians in the team working as care coordinators, psychiatrists and psychologists to:
  • Provide clearly defined recovery focused interventions, in line with patients’ care plans.
  • Actively participate in the Team’s meetings including Zoning, seeking to provide high-quality services strongly focused on recovery to patients
  • Positively promote and support independent living for patients in the community by maintaining extensive knowledge and links with the community resources and actively supporting patients to access them.
  • Enable patients acquire daily living skills: “living ordinary lives” (e.g. shopping and cooking).
  • Help patients gain access to resources and information about welfare benefits.
  • Help patients access physical health services and gain skills to monitor their health needs in the community on an on-going basis.
  • Help identify early signs of relapse by monitoring the patient’s progress, level of functioning and mental state, updating qualified staff involved in the care of the patient.
  • Support patients develop self-management tools and coping strategies.
  • Maintain adequate records as required by existing procedures; entering appropriate details in case records.

  • Following an assessment, to provide practical and recovery focused support to service users and carers as described in their care plan.
  • Working with care coordinators or lead clinicians to provide clearly defined recovery focused interventions, in line with care packages.
  • To attend and actively participate in team meetings/care plan review meeting and team training as appropriate.
  • To positively promote and support independent living for service users in the community by maintaining extensive knowledge and links with the community resources and actively supporting service users to access them.
  • To develop a rapport with service users based upon professional support and collaboration but within appropriate boundaries.
  • To provide regular and practical support to service users and carers to maintain dignity and independence.

  • To help service users to gain access to resources to include benefits and welfare rights.
About us

The Team is multi-disciplinary team who works in partnership with multiple other agencies: Adult Social Care, Housing Associations, Probation, Voluntary Sector.

The Team works from the Maddison Centre close to the vibrant Teddington high street and the river Thames with a range of shops, cafés and restaurants. The hub is located a short walk from Fulwell and Teddington stations.

They also see patients at Tolworth Hospital in borough of Kingston close to a number of shops and with good transport links close to the A3 and a bus ride away from Surbiton station, which has fast connections to Clapham Junction and London Waterloo.
