Position Overview & Specifications

We are seeking a dynamic,enthusiastic, and committed health visitor to join our friendly supportive FNP and vulnerable families team in Coventry.The team is part of a large integrated 0-19 service within Coventry’s’ Family Health and Lifestyles team (FH&LS).

The post holder will be working as part of the Coventry FNP team and the vulnerable families team focusing on a defined caseload area. The post holder will receive support, supervision and attend learning days from the FNP teams. There will be an opportunity to work alongside other early intervention health visitors within the trust.

This is a new role for Coventry and is an exiting opportunity for a qualified health visitor to work intensively with families.

To effectively lead and deliver on the Healthy Child Programme (HCP) 0-5 (DH 2010, updated 2021) alongside an extended programme to improve antenatal health, child health and development, and family wellbeing in young and vulnerable families within Coventry. These may also include Asylum Seekers, homelessness, refugees, sex workers, drug & alcohol addiction trafficking, temporary accommodation residential assessment unit and to work on both authorised and unauthorised sites for travellers.

Named responsibility and professional accountability for the management of a defined caseload.

To contribute to the Public Health Agenda, to reduce health inequalities within this caseload.

To actively lead and work as part of the team around the child working with partner agencies and utilising the various assets within the area of the caseload to meet the needs of the family.

To ensure that the service’s key performance indicators targets are achieved.

To act as a role model and be professional at all times adhering to the current NMC Code of Conduct.

To participate in nurse prescribing following NMC and local policy.

Act as a named Health Visitor for a defined caseload.

To plan and prioritise their own caseload and workload.

To lead and deliver on the Healthy Child programme 0-5, working with partner agencies to improve outcomes for children.

To use agreed and developed programme materials which fit with the SWFT Early Intervention programme through home visiting, clinic contacts in any setting, workshops, and use of technology, in order to achieve the following:

Improve the outcomes of pregnancy.

Improve children’s health and development by enabling parents to provide more sensitive and competent care of them.

Support parents with the transition to parenthood, and to overcome any barriers to achieving improved outcomes for themselves and their children including mental health issues, financial struggles, and anything else identified through assessment.

To work therapeutically with family members to promote behaviour change and positive outcomes for children and families.

Provide and receive highly complex, sensitive, or contentious information in respect of families on caseload.

To develop a caseload profile, assessing needs as Targeted, and Specialist. Early identification of children and families needing support through use of single assessment process and, if required, to include other members of the multidisciplinary team.

Act in accordance with current local Coventry Safeguarding Board and Trust policy to protect children at risk.

Promote and maintain good record keeping that conforms to NMC policy, data protection and Health Visitor record keeping standards.

Identify and report any hazards/ incidents relevant to the health and safety of clients or colleagues using the Trusts reporting procedure.

Adhere to the Trust lone worker policy and undertake risk assessments where needed in liaison with Supervisor.

To escalate any concerns as appropriate to your Clinical Lead.

Work closely with MAMTA to support BAME families within this client group

Provide an effective Health Visitor link role with GP practices, in particular specialist GP practices including the Anchor and The Meridian Centre and Community services to facilitate access for clients.

Knowledge, Skills and Experience

To be professionally and legally accountable for all aspects of own work ensuring a high standard of clinical care.

Demonstrate a good knowledge and understanding of relevant legislation, policies and procedures relating to child protection. Participate in risk assessment & decision-making process at child protection meetings, providing written reports, and health advice when required.

To participate in supervision, Child Protection, Clinical/ Restorative and Looked After Children and team learning.

To prescribe using the Nurse Formulary to ensure prescribing is undertaken in a safe, cost-effective manner.

Be flexible to the needs of the service.

Develop and maintain effective communication within the team, to include the wider health visiting service, to maintain productive working relationships and promote good morale within the team resulting in a quality service.

Ensure that all clients and colleagues are treated at all times in an equitable manner, respecting and showing an understanding of diversity in the workplace.

Ensure confidentiality is upheld at all times, as per Trust guidelines.

Participate in and contribute to team/ locality meetings to keep up to date with Trust information.

To be competent using Microsoft and other computer / phone applications as required.

To input patient contacts onto Trust computer system.

To ensure electronic work diary is up to date and all members of the team have full access to it.

To work collaboratively and respectively with other team members to support each other in achieving the aims of the service.

To take responsibility for your own and be supportive of others Health and Wellbeing.

Develop and maintain effective communication networks with service users, health care professionals and other statutory and voluntary agencies.

To work across professional and organisational boundaries to develop and maintain appropriate partnerships and networks to influence and improve outcomes for children and their families/ carers.

Raise awareness of health needs within families, using skills of negotiation and motivational interviewing to facilitate change leading to a healthier lifestyle.

Attend all meetings as appropriate, including video conferencing, to support the development of service delivery.

To work in an agile manner where possible, including working from base, home or other sites as felt beneficial to the service and individual, whilst recognising the need for continued team working and communication.
