Position Overview & Specifications

Social Worker in EL CAMHS Crisis & Intensive Community Crisis Service

The post holder will play an integral role providing the following to support to children, young people and families known to ICCS – and based in City & Hackney – as well as children and young people presenting in mental health crisis to Newham University Hospital:
  • Contribute to the multi-disciplinary assessment and intervention for children, young people and their families
  • Work collaboratively with professionals across health and social care to improve joint working and promote shared understanding of roles and responsibilities, to better support children and young people in mental health crisis
  • Work collaboratively in care planning with children, young people and families to ensure individual needs are met – such as from a cultural, religious, social background and disability perspective
  • To support safe and effective discharge from hospital, including liaison work with relevant agencies to ensure appropriate support is in place and provide direct input with follow up support at home or in the community
  • Liaise closely with CAMHS, hospital, social care and safeguarding colleagues to provide a holistic service for children, young people and their families
  • Support and advise colleagues in CAMHS through consultation and case discussion
  • To manage, plan and prioritise own clinical caseload and exercise autonomous professional responsibility for assessment and intervention, including risk management
To provide social care and systemic assessment and intervention of children, young people and their families – in hospital, clinic and home settings
  • To provide systemic interventions and parenting interventions for children, young people and their families in – in hospital, clinic and home settings
  • To provide mental health input to network of the child, young person or family as required
  • Work closely with relevant Social Care teams to ensure Tower Hamlets children and young people known to ICCS or presenting to Royal London Hospital are appropriately supported at the right time
  • Visit children and young people following their discharge to ensure support is in place and that they are receiving appropriate support
  • Contribute to planning meetings where risk, safety and placement may be in question
  • Coproduce care plans with children, young people, their families and relevant professionals – taking joint responsibility for assessment, treatment and discharge
  • Act as a liason between CAMHS and Children’s Service
This is an exciting opportunity for an experienced Social Worker to join the existing East London CAMHS Crisis Service and developing Intensive Community Crisis Service (ICCS) in an embedded role in Newham.

This post is fixed term for 23 months.
