High Intensity User Link Worker (Integrated Plus)

37 hours per week

Salary: £32,909 - £34,723 (aligned to NJC SO2)

This role will involve providing holistic one to one social prescribing support for people aged 18 and over who have high dependency on emergency services and who are frequent visitors/callers of A&E, the Urgent Care Centre and West Midlands Ambulance Service.

This role will also entail actively contributing as a member of a well-established social prescribing team who support the most vulnerable in society.

You will have experience of supporting vulnerable adults in a person-centred way and knowledge of strength-based recovery models and approaches.

Mileage paid for all posts at NJC rates where applicable.
