YOU
- Will support the development of an innovative service which offers transitional support and navigation for people who are struggling with being discharged from CPFT secondary services, and carers, and navigate to third sector providers and wider community resources as the need dictates.
- Will provide a coaching role to people receiving services, and their carers, ensuring effective transition from Trust services to the third sector and community resources. The Recovery Coach Team will also be required to negotiate rapid re-access to CPFT clinical services should the need arise.

To work in a co-produced and recovery focused manner assisting people receiving secondary mental health services to transition more seamlessly through statutory and third sector mental health services and into the wider community.
To work in a coaching manner, exploring ways of goal setting with people to help them achieve their ambitions.

- To support service users with transitioning from secondary care into primary care.
- To have a focus on supporting community-based practice, early intervention, and helping to develop strength and asset based, rather than deficit-based service models.
- To ensure good linkage to the Recovery Colleges and ensure that all interventions are in line with a recovery educational philosophy.
- Conduct outreach, promotion and capacity building work within designated communities to identify resources that may be of interest to people receiving services and their carers.
