Position Overview & Specifications
Funded by Midlands Partnership NHS Foundation Trust and working in partnership with Alzheimer’s Society, Burton and District Mind will provide an Outreach Service to support and encourage older adults (over 65) those at risk of Serious Mental Illness (SMI) within Underrepresented Community Groups, working in partnership with the Mental Health Teams within the Trust across the six districts of southern Staffordshire.
Outreach Workers will work with the following groups in identifying people with mental health services:
- Eastern European & South East Asian communities
- Asylum Seekers
- Co-occurring Substance Misuse
- LGBTQ+
- Travelling Communities
- Ex-offenders
Outreach Workers will provide a sensitive and informed approach to community engagement, building trusted relationships through networking with individuals, community groups and voluntary sector organisations.
Outreach Workers will coproduce recovery plans which will include access to MPFT’s Social and Community Hub (formally Access Team) for mental health assessments and further support, providing warm introductions and sustained support where required.
Salary at £20,872-22,143 (to be confirmed), 35 hours a week. Monday to Friday, 9am-5pm
Based at Burton on Trent and working from home. Must be Car Driver and willing to travel.
